There are two major systems to catalogue the severity of traumatic brain injuries. The Glasgow Coma Scale uses a scale from 1-15 to categorize damage to the brain and neural tissue. On the low-end of the spectrum (13-15), mild symptoms may result. Moderate TBI (9-12) results in impairment of cognitive function, comas longer than half-an-hour, and physical problems that may resolve on their own but may require rehabilitation. Severe TBI (3-8) often results in long-term coma. For the most severe TBI (under 3), the patient is essentially in a vegetative state and cannot respond to pain or interface with the environment in any meaningful way.

The Ranchos Los Amigos Scale also categorizes TBI on an 8-level scale based on the injured patient's cognitive abilities, ability to interact with the world around him or her, general awareness, and social behavior.
